چکیده

Pulsus alternans is a condition wherein the heartbeat cyclically varies between weak and strong despite a constant heart rate. It was first described many years ago in experimental conditions, but it can be observed clinically, in particular, in patients with heart failure, where the presence of alternans portends a bad outcome (reviewed in1). Typically, alternans occurs at high heart rates, but the threshold frequency can be lowered by various conditions such as ionic disturbances or ischemia. The alternation of the strength of the pulse, or mechanical alternans, is accompanied by alternation of the electrical activity, the so-called “electrical alternans.” This is particularly evident on the repolarization phase: action potential recordings in vitro show alternation of the duration of the action potential (AP), either concordant with the mechanical alternans (long AP with a strong beat) or discordant (short AP with strong beat) (reviewed in1). Electrical alternans can be observed at the ECG level as alternation of the ST-segment and T-wave amplitude and form; T-wave alternans is commonly referred to as TWA.

Dysfunctional intracellular calcium cycling in cardiac alternans

1. Cardiac alternans refers to a condition in which there is a periodic beat-to-beat oscillation in electrical activity and the strength of cardiac muscle contraction at a constant heart rate. Clinically, cardiac alternans occurs in settings that are typical for cardiac arrhythmias and has been causally linked to these conditions. 2. At the cellular level, alternans is defined as beatto-beat al...

Atrial SERCA2a Overexpression Has No Affect on Cardiac Alternans but Promotes Arrhythmogenic SR Ca2+ Triggers

BACKGROUND Atrial fibrillation (AF) is the most common arrhythmia in humans, yet; treatment has remained sub-optimal due to poor understanding of the underlying mechanisms. Cardiac alternans precede AF episodes, suggesting an important arrhythmia substrate. Recently, we demonstrated ventricular SERCA2a overexpression suppresses cardiac alternans and arrhythmias. Therefore, we hypothesized that ...

Cardiac alternans do not rely on diastolic sarcoplasmic reticulum calcium content fluctuations.

Cardiac alternans are thought to be a precursor to life-threatening arrhythmias. Previous studies suggested that alterations in sarcoplasmic reticulum (SR) Ca2+ content are either causative or not associated with myocyte Ca2+ alternans. However, those studies used indirect measures of SR Ca2+. Here we used direct continuous measurement of intra-SR free [Ca2+] ([Ca2+]SR) (using Fluo5N) during fr...

A model for cooperative gating of L-type Ca2+ channels and its effects on cardiac alternans dynamics

In ventricular myocytes, membrane depolarization during the action potential (AP) causes synchronous activation of multiple L-type CaV1.2 channels (LTCCs), which trigger the release of calcium (Ca2+) from the sarcoplasmic reticulum (SR). This results in an increase in intracellular Ca2+ (Cai) that initiates contraction. During pulsus alternans, cardiac contraction is unstable, going from weak t...

β-Adrenergic receptor stimulation inhibits proarrhythmic alternans in postinfarction border zone cardiomyocytes: a computational analysis

The border zone (BZ) of the viable myocardium adjacent to an infarct undergoes extensive autonomic and electrical remodeling and is prone to repolarization alternans-induced cardiac arrhythmias. BZ remodeling processes may promote or inhibit Ca2+ and/or repolarization alternans and may differentially affect ventricular arrhythmogenesis. Here, we used a detailed computational model of the canine...
